Job Summary:

Developmental Services of Dickson County is currently recruiting Family Model Applicants to support people with intellectual and developmental disabilities. Family Model is a service, in which, people supported reside in the applicant's home and the applicant provides their day-to-day care. Families are responsible for encouraging independence, personal care and the overall well-being of the person supported. Applicants will be responsible for completion of annual training, daily documentation and transportation of the person supported.

Requirements:

• Completion of periodic training programs.
• The employee must occasionally to frequently lift and/or move 50 to 75 pounds and maneuver a manual wheelchair.
• The employee is occasionally required to lift and/or move residents of any weight, with assistance of a Hoyer lift or other protective equipment, and use all equipment effectively.
• The employee must be able to handle highly stressful situations on a day to day basis.
• Able to pass a drug screen.
• Must be able to pass medication class, CPR training and First Aid training.
• Must be able to pass a home study and own or rent a home.

Minimum Qualifications:

• Must be at least 18 years of age.
• Must complete or test out of Agency personal care training.
• Must be able to perform all duties listed in assigned Individual Service Plans.
• Possession of a valid driver's license and access to a private vehicle for day to day job performance.

Education:

• High school diploma or GED preferred.
